4Kids4Families Celebrates One Year of Stage II in Foster Care Community-Based Care

East Texas – 4Kids4Families, the Single Source Continuum Contractor (SSCC) for Texas DFPS Region 4, is proud to celebrate one year of successfully implementing Stage II of Community-Based Care (CBC) for foster care. This milestone reflects a major step forward in the state’s effort to shift the foster care system from a state-run model to one led locally by community-based providers.

Launched in May 2024, Stage II of foster care CBC expanded 4Kids4Families’ role to include case management services for children and youth in state conservatorship, in addition to managing placements and support services. Now, children and families in 23 East Texas counties are served directly by people in their communities, those who best understand the local context and needs.

“This first year of Stage II has proven the power of proximity,” said Jon Mark McMullen, Executive Director of 4Kids4Families. “By moving decision-making closer to children and families, we’re seeing more stability, faster responses, and more personalized support across the foster care system.”

Key outcomes during the first year of Stage II implementation include:

  • Localized case management that allows for quicker, more informed decisions for children in care
  • Increased collaboration with local foster parents, providers, and community organizations
  • Expanded placement stability and support for kinship caregivers
  • Enhanced wraparound services focused on healing and family reunification

 

Community-Based Care is a foster care model designed to improve outcomes by giving local communities the flexibility and accountability to provide children with the right services at the right time.

“We’re grateful to every caseworker, caregiver, and community partner who has embraced this shift with us,” said McMullen. “Together, we are building a foster care system rooted in compassion, connection, and community.”

Region 4 includes the following counties: Anderson, Bowie, Camp, Cass, Cherokee, Delta, Franklin, Gregg, Harrison, Henderson, Hopkins, Lamar, Marion, Morris, Panola, Rains, Red River, Rusk, Smith, Titus, Upshur, Van Zandt, and Wood.

Reportable Incidents Include:

  • Death of a child
  • Child is in a life-threatening situation
  • A significant change in a child’s medical condition
  • Psychiatric hospitalization
  • A child who is missing from care and placed on the Amber Alert system returns to care
  • Natural disasters where children are displaced
  • Suicide attempts
  • Injuries requiring medical treatment including psychiatric hospitalization
  • Runaway incidents
  • A caregiver/staff member or child contracts a communicable disease
  • Commission of a Crime, including those committed by youth or crimes occur at a home or facility in which youth are placed
  • Allegations of abuse, neglect or abusive treatment
  • A child’s abuse, neglect, or exploitation.
  • Allegation and/or confirmed child-on-child physical and/or sexual abuse.
